We’re looking for an Independent Assessor and Prescriber to join our exciting ADHD service for 37.5 hours per week. This is a fantastic opportunity to make a difference from the outset and play an important role in helping us develop and deliver the service.

Main duties of the job

As an Independent Assessor & Prescriber you will work as part of the PCS ADHD service working across the interface between primary care, voluntary sector and secondary services, with the aim of improving the mental health and well-being of our communities.

You will support individuals to move from surviving to thriving by helping people to feel in control of their health and wellbeing, by linking service users with health and social groups and pathways to address the impact of living with undiagnosed ADHD symptoms.

You will be integrated as part of the primary care ADHD infrastructure, providing ADHD Assessment, Diagnosis and prescribing treatments.

Job responsibilitiesClinical Practice

• Deliver structured diagnostic assessments (history, impairment, co-morbidity, risk) and provide clear feedback and written plans.
• Conduct structured follow-up reviews to monitor treatment response, adjust care plans and ensure safe, effective long-term management.
• Work with the pharmacy team to initiate and adjust medication within the local formulary and arrange baseline assessments and ongoing physical health monitoring.
• Conduct brief titration contacts until the patient is stable.
• Complete safe shared-care transfers to primary care using the Electronic Prescription Service and relevant templates.
• Document clinical activity promptly and accurately, produce high-quality letters on the day, and maintain accurate and auditable records.
• To maintain high standard of care in all areas of practice in accordance with Local and National standards.
• Maintain and develop professional knowledge, skills and expertise, acting at all times in accordance with professional standards and ensuring practice remains responsive to changing care needs.
• Work within the scope of professional competence and autonomy, recognising when guidance, support or escalation is required.

Leadership and Service Development

• Present complex primary care cases at the weekly MDT and contribute to audit, incident learning and quality improvement.
• Support the induction of colleagues, participate in case discussions and provide day-to-day coaching, while upholding safeguarding, consent and capacity, and information governance requirements at all times.
• Support the effective functioning of the service through leadership tasks as required and use leadership skills to support the wider multi-professional team.
• Contribute to the development of procedures and undertake delegated tasks as required.
• Supply the necessary information of activity outcomes (and other KPI data) as agreed with the Service Team Manager and Head of Service.
• Use IT skills appropriate to the needs of the post and contribute to the use of new IT tools and reporting systems, including the emerging use of AI.
• Exercise professional judgement to promote safety when managing complex or unpredictable situations, demonstrating effective teamworking, resilience and determination, and ensuring concerns are appropriately escalated.

Person SpecificationExperienceEssential

• Experience working with adult's with ADHD
• In-depth knowledge of the barriers people with health inequalities experience
• Working knowledge of NICE ADHD guidance, local formulary, risk and safeguarding

QualificationsEssential

• Registration with one of the following professional bodies: NMC, GPhC or GMC with Independent Prescriber Annotation
• UKAAN-trained

Skills and CompetenciesEssential

• Excellent communication and interpersonal skills, with the ability to build effective relationships with service users
• Ability to work on own initiative without direct supervisor, understanding where clinical input is needed
• Ability to identify risk and manage appropriately within the scope of practice
• Ability to plan and provide clinical structured intervention
• Ability to use clinical data/KPIs to monitor service activity and outcomes
